Can someone tell me a) what caused this, how to prevent it and how to fix it if possible.

You should be fine, I had the same thing happen to my plant on the Grobo start.
Next time try to poke a deeper hole on the peat pod like @Stephen explained on the video. I’ll make sure not to forget, since I did the first time.

i would cover that root hole up with a bit of stuffing / pod piece. Otherwise it will stand up on its on and eventually be a solid branch coming up. No need to worry!
